Public forms collect the information and documents you choose to submit, along with limited technical information used for security, abuse prevention, auditing, and investigation. Submit only what the office needs to review your request.
Use and retention
Submissions become an auditable office intake record. Authorized personnel may review them, request additional information, accept or reject a request, create related office records, and preserve communication history under office retention policy and applicable law.
Confidential information
Do not upload a confidential domestic-violence affidavit through a general public form. Confidential filings and court-controlled records must use the process directed by the court. Public status information is limited and is not the official court docket.
